Financial Hardship & Compassionate Care
SMEMPS offers fee waivers, fee reductions, and payment plans for patients who cannot afford their ambulance charges. Eligibility is based primarily on household income at or below 300% of the Federal Poverty Level.
To apply, complete the EMS and Ambulance Transport Hardship Waiver, Fee Reduction, and Payment Application and submit it with the required income documentation (recent pay stubs or bank statements) to Wittman Enterprises. A separate application is required for each request.

Requesting Records
Patients
To request your own records, call Wittman's Patient Financial Services Department at 1-800-906-6552. A HIPAA release form will be required.
Attorneys & Copy Services
Submit medical or billing record requests through Chart Swap. Make sure your request is directed to the service provider, SMEMPS.
